How to make an appointment
Generally, patients are referred to The Center by a doctor who has diagnosed or would like to rule out cancer or a disease of the blood. Your referring doctor will send us your test reports, demographics sheet, and insurance information. After we receive your information, we will call you to schedule an appointment.
We also encourage self-referred patients who are seeking second opinions. A doctor will examine you and/or review your records and provide you with their professional opinion. After that, you are welcome to either continue treatment with your current doctor or to come to The Center. Please call us at one of the numbers to your right, or call our main number: 856-424-3311.
What should I bring to my appointment?
Insurance Card(s) (this includes your current Medicaid card if you have Medicaid coverage).
Co-pay and referral (if applicable). Please note that all of our doctors are specialists and you will require a referral from your family doctor if you have an HMO.
Copies of recent test results if available.
A list of questions for the doctor.
It often helps to bring a friend or family member to your visit so that you have help remembering everything that the doctor says and to provide moral support.
New patients should bring a completed New Patient Packet.
Please inform the receptionist upon arrival if your address, phone number, or insurance has changed since your last visit. If you had tests done outside of The Center, please ask the lab to send the report here so your doctor may review them and the staff may keep your file up-to-date.
